The Coordinator of Support Services will outreach, engage, promote and provide services to increase connectivity with WIR graduates and terminations to ensure a continuum of care and determine the level of services and support needed. Will work a flexible schedule and be required to work evenings and weekends as needed. • Assists graduates in supportive programming to increase connectivity, engagement, and meet basic needs that support successful integration into the community in a culturally responsive way, including: • Provide support services for graduates of WIR and their families including: outreach, information about community resources, educational groups, parent training, and basic employment services/skills. • Effectively document group progress notes in agency electronic health records within agency policy and procedure. • Attends and participates in required department and agency meetings and trainings.
