Outreach Coordinator and Event Planning

The Coordinator of Support Services will outreach, engage, promote and provide services to increase connectivity with WIR graduates and terminations to ensure a continuum of care and determine the level of services and support needed. Will work a flexible schedule and be required to work evenings and weekends as needed. • Assists graduates in supportive programming to increase connectivity, engagement, and meet basic needs that support successful integration into the community in a culturally responsive way, including: • Provide support services for graduates of WIR and their families including: outreach, information about community resources, educational groups, parent training, and basic employment services/skills. • Effectively document group progress notes in agency electronic health records within agency policy and procedure. • Promote individual, family, and group services that support long-term • Reduces barriers and arranges referrals, service coordination to CCBHC Continuing Care or other agency mental health, trauma and family services, professionals, agencies, community programs, or other appropriate resources to meet client needs.
