Description: SUPERVISES: None The Case Manager and Discharge Planner is responsible for coordinating pre-release or post-release care management and assessing the discharge planning needs of all patients The Case Manager and Discharge Planner will also coordinate community services to ensure successful re-entry into society Provides case management and discharge planning services for individuals , including pre- and post-release interviews and coordination of community resources Ensures continuity of care by coordinating medical benefits, social services, and outside behavioral health/substance abuse services, as well as obtaining medication scripts may assist in the job shadow portion of the interview process and orientation/training of new employees. Documents relevant clinical information and data for quality initiative purposes to improve patient care Individuals in this position will have skills to actively listen and demonstrate understanding to best meet the patient’s individual needs. Due to the unique challenges of the target population individuals serving in this role will need good problem-solving skills.
